- Jo-Beth and Jo-Barf have the exact same gesture, but Jo-Barf's gesture dialogue is altered.
- She reuses Jo-Beth's Future Perfect voice clips, but they are pitched down and distorted.
- Jo-Barf's fire scream is an altered version of one of Jo-Beth's TSFP death sounds.
- There are many physical differences between her and Jo-Beth. She's much taller, her skirt is not opened, her teddy bear's head is ripped off, "hate" is written on her shirt instead of "SLUT", her boots are smaller and only have two straps, her backpack's straps are brown and studded instead of blue, her collar is fused into her neck leaving protruding spikes, her hands are deformed and lack fingers, and her clothes are lacking in saturation. She is also mirrored compared to Jo-Beth.
- Regardless of being a reversed Jo-Beth and having no hands, her first person arms are still the same as Jo-Beth Casey's.
- Her internal name is doppelganger.
