Headsprouter appears in TimeSplitters Future Perfect as a playable character. The character has no eyes or nose; just a large mouth with big teeth. Headsprouter's bone arms don't have any hands - just hooks - he wears dark brown trousers and his chest is ripped open.
Headsprouter appears in the Story level What Lies Below. He hangs off the roof of the mine when you are travelling in a mine cart and explodes when you shoot him, causing damage if you are close. It is possible that Headsprouter was once a zombie who was decapitated, and then a strange mutation grew in the head's place.

- It reuses voice clips from the Insect Mutant.
- Headsprouter might be Jacob Crow's first attempt at creating the physicality of a TimeSplitter. Headsprouter, like the Berserker Splitter, sports large teeth, no visible eyes, similarly textured skin around its collar and neck as well as skinless forearms. This is consistent with the Zombies that "control energy", also seen in the labs. The next stage of this process would be The Freak, who also possesses explosive tendencies.
- It shares its viewmodel arms with the Deerhaunter and Carrion Carcass.
